A Life Celebration Questionnaire
We are honored that you have chosen to collaborate with us on the creation of your loved ones special and truly one of a kind memorial.
By taking the time to answer these questions you are helping us in the process of specially designing the most meaning filled and unique memorial.
Important
On the day of your In-Person Discovery Meeting, please bring:
- Three of your favorite photos of your departed loved one.
- Three small personal artifacts of your loved one (fit into a shoe box)
- Two written out words of meaning (quotes, sayings, song lyrics, etc.)
- The above questionnaire - completed.
We will return your photos and small artifacts on the day we set up your memorial table.
We are here to help you and your loved ones with this meaningful life process.
